I, _________________________ (consignor/buyer/adopter] have hired Beverlee Dee to transport the horse I have purchased/adopted to the physical address above. I will be responsible for arranging to have either myself of someone knowledgeable in the handling of horses/the owner/manager of the boarding facility (if horse will be boarded) available to meet the horse when he/she arrives.
Beverlee Dee, Bright Futures Farm, their agents, affiliates, employees, representatives of all types, officers, directors and their successors and assigns is/are held harmless from liability in the event of an accident during transport, loading or unloading of the aforementioned horse. Every effort is made to maintain a safe environment and safe travel conditions for each horse in our care. While we do not anticipate any problems, accidents do occur, and therefore Beverlee Dee, Bright Futures Farm their agents, affiliates, employees, representatives of all types, officers, directors and their successors and assigns will not be held responsible for any personal and/or property damage that may occur during loading, transport and unloading of the aforementioned horse. Furthermore, Beverlee Dee, Bright Futures farm, their agents, affiliates, employees, representatives of all types, officers, directors and their successors and assigns will not be held responsible for any injury to the aforementioned horse being transported if injury should occur during transport, loading or unloading of the aforementioned horse.
I understand that the cost of transport is $__________________, and that all monies agreed to must be paid in full prior to unloading of the aforementioned horse.
I understand that if the horse is to wear polos, boots and/or blanket that the horse is to be dressed prior to the arrival of the trailer.
I understand that there is a fee of $40 per hour payable in fifteen minutes increments if the horse does not load within thirty minutes.
I understand that there is a fee of $40 per hour payable in thirty minute increments for waiting if I have not secured the balance of monies prior to pickup/arrival of the aforementioned horse.
I have read, understand and ascent to the terms and conditions herein.
